How Does Dr. Rad Approach Rhinoplasty Surgery?


Rhinoplasty is a delicate procedure requiring skill and experience to restore proportion, and Dr. Rad is one of the few plastic surgeons offering the “closed” scarless nose job. Very few surgeons can perform scarless rhinoplasty. However, the advantages of scarless techniques are many: aside from achieving equally excellent results, Dr. Rad’s closed technique involves no visible scar across the base of the nose, minimal invasiveness, limited swelling, and short recovery (7 days).

For your comfort and safety, light general anesthesia is administered by a Board-certified anesthesiologist in a hospital operating room and a breathing tube protects your airway. Dr. Rad makes incisions hidden inside the nostrils, not across the base of your nose as in the “open approach,” then he creates a precise tunnel through which a dorsal hump can be reduced and straightened. Dr. Rad then borrows cartilage from the septum (thus straightening a deviated septum, called septoplasty) and uses it as graft material to make refinements in nasal shape. When extra cartilage is required, the ear or rib are good sources. Dr. Rad places cartilage grafts in key areas to enhance and refine the shape and also improve airflow. Dr. Rad narrows the nasal bones and nasal base when necessary. Splints are placed inside and outside the nose for approximately 1 week, after which time patients can return to work and most activities with little restriction.

How Is Rhinoplasty Done?

Dr. Rad uses a closed “preservation” rhinoplasty approach. “Closed” means that skin is left mostly intact over the underlying nasal skeleton (not separated), and “preservation” means that key ligaments (scroll and Pitanguy) are not sacrificed. The combination of closed preservation rhinoplasty is focused on minimizing invasiveness. Dr. Rad makes small incisions within the nostrils which gives him access to the bridge, tip, nasal bones, septum and turbinates. Dr. Rad accesses only the structures that require modification and by “preserving” ligaments and the soft tissue envelope, nasal stability is maximized. Also, avoiding excess surgical dissection reduces tissue trauma, swelling and bruising and recovery time is 1-2 weeks.

Open rhinoplasty is done through nostril and columellar incisions, the overlying skin is separated from underlying structures, and then cartilage, nasal bones and septum are exposed. The surgeon modifies those structures to change their shape, and then places the skin back down. This approach is called “structural” rhinoplasty and is different from closed preservation. Open rhinoplasty may be useful in difficult corrective rhinoplasty.

WHAT IS A RHINOPLASTY?

CAN RHINOPLASTY IMPROVE MY BREATHING?

About 80% of people have a deviated septum, large turbinates or some other anatomical reason for impaired airflow. Poor nasal airflow impairs sleep which can lead to fatigue, lack of concentration and poor work performance. Rhinoplasty and septoplasty can significantly improve airflow through nasal passages which can improve sleep and restfulness. Even in the absence of a deviated septum, Dr. Rad employs airway preserving techniques, called “dorsal preservation”, as well as other methods such “spreader grafts” (cartilage pieces placed in the internal nasal valve) to open the airways further. Improved breathing and beautiful aesthetic results are always Dr. Rad’s goal for his patients.

WHAT IS A NON-SURGICAL RHINOPLASTY?

Non-surgical rhinoplasty is an in-office procedure whereby Dr. Rad injects dermal filler in precise areas of the nose to improve nasal shape and proportion. It is ideal for patients who have volume deficiency in specific areas. The ideal candidate for non-surgical rhinoplasty are those who have a dorsal hump or downward curving shape of the nose and for whom the radix (upper bridge at eye level) is concave. Dr. Rad carefully studies and measures nasal proportions, and he also pays attention to patients’ ethnicities, in order to optimize nasal shape. With topical numbing medicine pain is minimal, no recovery time is necessary, and results are immediate. The results last 6-12 months depending on the filler used and the amount of volume placed. Dr. Rad uses hyaluronic acid-based fillers such as Juvèderm, Voluma, Vollure, or Radiesse depending on patient characteristics and desired goals.

WHY DOESN’T EVERY SURGEON PERFORM CLOSED RHINOPLASTY?

There are many methods for rhinoplasty and choice of technique depends on many factors. Surgeons-in-training are usually exposed first to the open method because the nasal skeleton is visible during surgery, whereas closed rhinoplasty can be challenging to learn because of limited visibility (by design to minimize invasiveness). However, many skilled rhinoplasty surgeons, especially those who perform rhinoplasty frequently, are capable of open and closed methods. While Dr. Rad has expertise in open methods, he chooses to specialize in closed and preservation techniques which gives patients access to the full spectrum of rhinoplasty surgery.

IF I’VE HAD A RHINOPLASTY BEFORE AND I NEED FURTHER IMPROVEMENT, AM I ABLE TO UNDERGO RHINOPLASTY AGAIN?

Primary (1st time) and revision (2nd, 3rd or more) rhinoplasty patients can have an improvement, and this depends on many factors. In revision rhinoplasty, typically open or closed (endonasal) approaches are used, and ear or rib cartilage graft is needed. These sources of cartilage are essential because the septum likely has been used in prior surgery and no longer has the additional cartilage needed to correct the shape. There is no “one size fits all” approach in revision rhinoplasty and the specific plan develops through careful consultation to assess your anatomy, challenges, goals, downtime and probability of success.

HOW DO I CHOOSE A RHINOPLASTY SURGEON IN WASHINGTON DC?

Particularly in rhinoplasty, surgery can’t be reversed and correcting inexperienced work is more challenging (but not impossible), so choose right the first time. When vetting a plastic surgeon, use this checklist: S/he should be certified by the American Board of Plastic Surgery, trained in an accredited (by ACGME) program at renowned institutions, have years of experience performing rhinoplasty regularly (every week), show extensive before/after photos of patients, and the surgeon’s aesthetic should resonate with you. WHAT IS SEPTOPLASTY?

The septum is a flat piece of cartilage that is the barrier between your twin nasal passages. Sometimes the septum is bowed to one side (called “deviated septum”), causing airway obstruction. A septoplasty is a surgical procedure Dr. Rad performs to (1) straighten the septum to provide better airflow in patients who are suffering from a deviated septum, and (2) obtain cartilage grafts which Dr. Rad uses to reshape the nose for cosmetic improvement. Dr. Rad performs rhinoplasty and septoplasty together. If you’ve already had a septoplasty in the past and now want cosmetic improvements to the shape of your nose, then Dr. Rad may need to borrow cartilage from other areas (the inner “bowl” of the ear, or a rib) to obtain enough cartilage for grafts.

SHOULD OTHER PROCEDURES BE CONSIDERED WITH RHINOPLASTY?

If you have concerns for improving other areas of the face then Dr. Rad will advise you. While rhinoplasty can improve the appearance of the nose itself, facial balance and proportion are crucial to consider. Dr. Rad will evaluate your facial architecture to determine if you would benefit from enhancement or refinement of other areas of your facial skeleton. The most popular adjunct to rhinoplasty is chin augmentation (genioplasty) for a receded or diminutive chin. When the chin is too small the nose often looks disproportionately large. Chin augmentation can improve the projection, shape, and size of the chin. In combination with rhinoplasty, the facial balance can be improved. Facial width is also important to consider: some patients benefit from enhancement of the cheekbones either through facial fat grafting or malar (cheek) implant augmentation, whereas others may benefit from slimming of “chipmunk” cheeks via buccal fat pad removal. Patients in later years may combine facelift or neck lift surgery with rhinoplasty. The bottom line is that combining surgeries is generally safe and requires careful assessment of your facial architecture and your goals. Overall combining procedures are generally safe, cost-effective, and involves one recovery period, making it a popular approach.

HOW ARE COMPLICATIONS AVOIDED?

WHAT IS PRESERVATION RHINOPLASTY?

Preservation Rhinoplasty is a minimally invasive approach that was initially proposed over a century ago and has recently resurged in importance. The philosophy is centered on preserving, rather than dissecting or cutting through, nasal structures, specifically ligaments, the soft tissue envelope and cartilage. The idea is that minimal disturbance of the intrinsic, native structures that provide support to the nose will preserve stability. Can preservation rhinoplasty offer equally good results to other methods? Absolutely yes. This is why it has gained tremendous interest because minimizing invasiveness and achieving beautiful results should always be prioritized.

There are two main components: dorsal preservation and tip preservation. “Dorsal” refers to the “dorsum” or the bridge of the nose. Commonly patients who desire rhinoplasty wish to have a “hump” or convex bridge reduced to either a straight or slightly curved appearance. There are different methods of doing this. The traditional approach is “component dorsal reduction” which basically means removing (cutting off) the top of hump. Preservation methods use a “push down” or “let down” which involves shifting the hump downwards without cutting off the top. Tip preservation means that we are preserving most of the ligaments that support tip shape, shaping the tip to the degree necessary, and then re-attaching the ligaments. The ligaments that are crucial to preserve are the “scroll ligament complex” and the “Pitanguy ligament”. By repairing these ligaments after tip shaping, the surgeon has more control over the supra tip break point, lateral alar definition and columella position. There are many technical details about preservation and closed methods, however the most important takeaway for patients is that Dr. Rad’s goals are focused on achieving best results while minimizing surgery and preserving native structures. This gives patients the best chance for enjoying a beautiful and functional nose with minimal risk of complications.
